The aim of this study was to investigate the effects of a ketogenic diet on the electroencephalogram and seizure frequency in patients with drug-resistant epilepsy. The study enrolled nine children with epilepsy who were treated with a ketogenic diet for a minimum of 6 months (range 6-9). The primary outcome was reduction of seizure frequency and epilepsy after 12 months. Secondary outcomes included the duration of seizure freedom, electroencephalographic alterations and toxicity. The study period was 12 months. The participants received a total of 28.5 g/day of protein and 61.5 g/day of fat. Patients received 600-800 kcal/day of fat. The mean reduction of seizures was 40.6% and the mean reduction in seizure frequency was 89.7%. The mean duration of seizure freedom was 10.2 months. During the diet, three patients had spikes, six patients had periodic complexes and one patient had unilateral posterior dominant spikes. Two patients reported minor gastrointestinal symptoms; one patient reported constipation and one patient reported nausea and vomiting. There was no serious toxicity in this study.
